If your student has EVER attended one of our schools you do NOT need to complete a Pre-Enrollment. Please contact your school of enrollment.
Requirements to Register New Students
2025-2026 School Year
- Students enrolling in Kindergarten must be five years of age by September 1, 2025
- Students enrolling in Transitional Kinder must turn four years of age by September 1, 2025.
- Preschool students must be at least 3 years of age, must turn 4 years of age on or after September 2, 2025 and be potty trained.
2024-2025 School Year
- Students enrolling in Kindergarten must be five years of age by September 1, 2024
- Students enrolling in Transitional Kinder must turn five is between September 2, 2024 and June 2, 2025.
- Students enrolling in preschool must be five on or after June 2, 2025, must be at least 3 years of age and potty trained.
Pre-enrollment information may be filled out in any other language by selecting the language tab at the top right
Please upload the following documentation in Pre-enrollment. You may also present the documents in person to your corresponding school.
1. Verification of Student Date of Birth:
✓ Original Birth Certificate, Passport, Certified Hospital Birth Record, or duly attested Baptismal Record.
2. Verification of Immunizations - Your child must have all required vaccinations. The following immunizations are required for school entry: If you are unable to upload this information, verification of immunization must be presented to school office personnel
✓ 5- DTap, 4- Polio, 3- Hepatitis B, 2- MMR, 2- Varicella.
✓ Tdap is required for students enrolling in the 7th grade.
3. Proof of Residency (ONE of the following is required) - Document must be dated within 30 days:
✓ Gas, Water, Electric, Telephone, rent receipt or a lease agreement. Bill must have parent’s name and date on the document. If unable to provide proof of residence, ask for an Affidavit of residence and return with a proof of address of person who signed the affidavit.
4. Oral Health Assessment:
✓ Only for Kindergarten
5. Legal Custody papers – if there are any legal court orders or foster placement papers.
6. Special Education:
✓ If your child has an IEP individualized Education Program
7. Physical Examination (CHDP):
✓ Is required for students enrolling only in Kindergarten or Transitional Kindergarten.
